Make a Claim - Instructions

To qualify for payment, you must be an eligible Settlement Class Member and submit a valid Claim Form by August 10, 2023.

If you do not have your Unique ID, you may file a paper Claim Form, available here.

File a Claim by Mail:

You may download a paper Claim Form here or by calling 1-844-451-2728 to request one be mailed to you.

You should read the instructions on the Claim Form carefully and fill out the entire Claim Form. You also must sign your Claim Form under penalty of perjury.

If you have any questions relating to the Claim Form, you can call the Settlement Administrator or Class Counsel.

You must mail the Claim Form postmarked no later than August 10, 2023, to the following address:
